The care manager answers
The enquiry is handled by a care manager who asks what has changed, what the person needs and what the family is worried about.
Free face-to-face assessment
The care manager visits the person at home. This allows the assessment to consider the environment, routines, mobility, relationships and what happens between visits.
Care plan and matching
The care plan is created from the assessment. Carers are selected according to the client’s needs, required competencies, personality and preferences. The family can meet the carer first.
The first week is hands-on
A senior member of the team attends with the carer during the first week to show the routine, observe what works and tailor care to the person.
Ongoing review
Care is reviewed whenever the person’s condition changes and at least every six months. Families contact the care manager if something is wrong.
